Four people read this document, and each of them wants a different line out of it.

  • Reliability engineer

    You are calculating intervals and failure rates. The meter readings at start, finish and last service are what make mean time between failures a number rather than an estimate.

  • Maintenance planner

    You are scheduling against criticality and stock. Work type, criticality and the parts used decide what gets a PM, what gets a spare on the shelf, and what runs to failure.

  • Maintenance technician

    You are the one filling it in, often on a permit. The isolation rows and the zero energy state proved line are what protect you, in that order.

Where an equipment work order is used

The document is the same shape everywhere. What changes is which fields carry the weight.

What an equipment work order should include

An equipment work order is the record that authorises work on one identified asset and documents the readings, the safety steps and the work performed. It names the equipment by ID, captures the meter reading before and after, evidences the isolation and permit steps taken, and closes with the parts, labour and the failure detail.

Fields specific to an equipment work order

If you cut the form down, keep the meter reading at start. Everything reliability-related is built on it: service intervals that hold, mean time between failures, and the ability to say whether an asset is being worked hard or simply badly maintained. Taken at the finish only, it tells you when you left and nothing about what happened.

How do you fill in an equipment work order?

Fill it in the order the job happens, and take the meter reading before you touch the machine. Six steps, and the third is the one that is skipped when the line is down.

  1. Identify the asset by ID and by plate

    Asset ID, parent system or line, manufacturer, model and serial number. A description is not an identity, and two machines described the same way is how history gets mixed.

  2. Set work type, priority and production impact

    Corrective, preventive, inspection, modification or commissioning, with the criticality from the asset record rather than from the mood of the shift.

  3. Take the meter reading before you start

    Running hours, cycles, production count or energy, whichever the interval is based on. Taken afterwards it is worthless for calculating anything.

  4. Work the isolation and permit steps in order, and time them

    Risk assessment, permit, electrical and mechanical isolation, stored energy released, locks and tags applied, then prove the zero energy state. Record who and when against each.

  5. Do the work and record the failure detail

    What was found, what was done, parts with the warranty question answered, and labour. A part fitted under warranty still needs its line.

  6. Return to service, test it, then remove the locks

    In that order, and take the closing meter reading. Locks removed before the test is the sequence that causes incidents.

The history is not a separate document to maintain. It is what a run of work orders becomes, provided each one carried the asset ID and the meter reading. Work orders filed against descriptions rather than IDs produce no history at all, which is why the identity fields matter more than they look.

Where a document stops working

A form handles one job well. It handles an asset base badly, and the failure modes are consistent.

  • There is no version control

    Somebody revises the isolation steps, prints the form, and two procedures are in use on the same site. Nothing tells you which job followed which.

  • Meter-based intervals cannot run

    A service due every five hundred hours needs the readings compared automatically. On paper nobody compares them, so the interval quietly becomes whenever someone remembers.

  • The permit record is not searchable

    An isolation block on a sheet is compliant only in theory. After an incident the question is every isolation on that asset for two years, and that is a folder search.

  • You cannot compute reliability

    One form describes one job. Mean time between failures, cost per asset and which model fails earliest exist only across forms, and a folder cannot calculate them.

What is an equipment work order?

An equipment work order is the record that authorises work on one identified asset and documents what was done. It captures the asset ID and plate detail, the work type and criticality, the meter reading before and after, the isolation and permit steps, the parts and labour, and the failure detail.

It differs from a general maintenance work order in the identity and the meters. Reliability work is built on being able to attribute every job to one machine and one reading.

What should an equipment work order include?

At minimum: a unique work order number, the date raised and by whom, the work type, the priority and production impact, the requested completion, the equipment ID and parent system, the manufacturer, model and serial number, the criticality, the warranty status, meter readings at start, finish and last service, the full isolation and permit record, the parts and labour, the failure detail, and both signatures.

Why take a meter reading before starting?

Because everything interval-based depends on it. A service due every five hundred running hours cannot hold if the reading is only captured at the end, and mean time between failures cannot be calculated at all.

It also tells you whether a failure was early. Four hundred and sixty-two hours into a five-hundred-hour interval is a different conversation from nine hundred.

What does criticality do on the form?

It decides response before anyone forms a view on the day. Held against the asset as A critical, B important or C routine, it makes prioritisation consistent between shifts and between people.

Argued per job instead, priority tracks whoever is loudest rather than what the business actually depends on.

Why is the isolation and permit block so long?

Because each line is a separate thing that has to be true: the risk assessment reviewed, the permit issued, electrical and mechanical isolation applied, stored energy released, locks and tags on, and the zero energy state proved.

Proving the zero energy state is the one that is skipped under time pressure, and it is the one that makes the difference between an isolation and an assumption. Locks come off after the test, never before.

What is the difference between an equipment work order and an asset service history?

The work order records one job. The service history is what a run of work orders becomes when each one carried the asset ID and the meter reading.

The history is not a separate document to maintain. Jobs filed against descriptions instead of IDs simply produce no history.
